A hospital discharge can feel overwhelming when there are care needs, housing arrangements and NDIS supports to organise at the same time. Our Hospital Discharge Program is designed to make that transition smoother and safer for participants, families and hospital teams.
We help look at what support is needed after discharge, what risks need to be managed and which accommodation option may be suitable. This may include Short Term Accommodation for temporary support or Medium Term Accommodation when a participant needs somewhere to stay while longer term arrangements are being finalised.
Ritz Care was founded by Registered Nurses, and our clinical background plays an important role in how we support participants after hospital. We understand that recovery does not stop at discharge, especially when someone has complex health needs or requires extra help with daily living.
Our team can assist with personal care, community nursing, medication support, daily routines, meal preparation, transport and coordination with other providers. With RN oversight, we aim to recognise concerns early, support continuity of care and reduce the risk of preventable hospital readmissions where possible.
