Our community nursing support services provide reliable clinical care in the comfort of your home. This may include wound care, medication administration, continence support and general health monitoring.

Delivered by qualified nurses who understand the importance of both skill and compassion, this service helps you stay on top of your health without the stress of hospital visits.

Participants often feel more confident managing long-term conditions and benefit from regular check-ins that promote stability, safety and peace of mind. We aim to make healthcare more accessible, responsive and connected to the everyday needs of those we support.

Supported Independent Living

Our Supported Independent Living support services offer a safe, stable environment where participants can live with greater confidence and autonomy.

We assist with daily routines, personal care, community access and other tasks that help each person live as independently as possible. Living arrangements are carefully matched to individual needs, creating a home that feels supportive and secure.

These services often help participants build new skills, form meaningful relationships and enjoy a greater sense of control over their lifestyle. With the right support in place, independence becomes more achievable and sustainable over time.

Community Participation

Our community participation support services help participants stay socially active, build relationships and take part in meaningful activities. This could include joining a class, attending events, or exploring local interests. These experiences often improve confidence, reduce isolation and create a sense of belonging.

We work closely with participants to find opportunities that feel comfortable, enjoyable and suited to individual goals. Whether it’s a quiet outing or a busy event, our support focuses on building routine, connection and confidence outside the home. This kind of engagement often becomes one of the most valued parts of a person’s week.

Group & Centre Activities

Group and centre-based support services offer structured, welcoming environments for participants to learn new skills and enjoy social interaction. Activities might include cooking groups, craft sessions, games, or fitness programs, depending on interests and goals.

These supports are a great way to meet others, practise communication skills and build a sense of routine. Participants often find that shared spaces allow them to express themselves, gain confidence and feel part of a supportive community. Our team creates inclusive spaces that are relaxed and safe, with the right balance of support and independence.

Innovative Community Participation

This service encourages participants to try new things, discover personal interests and develop social and professional skills in creative ways. Innovative community participation support services are more flexible and tailored than traditional models, allowing for customised activities based on individual preferences.

From mentoring programs to volunteering or skill-based workshops, this support often leads to growth in confidence, self-expression and purpose. Participants may work on communication, independence, or simply enjoy new experiences with guidance from our team. It’s a fresh approach to community involvement that helps people feel more engaged and empowered.

Getting to appointments, work, social events or daily errands is easier with our travel and transport support services. We offer safe, accessible transport with staff who understand your routine and preferences. This support gives participants greater independence and flexibility in how they spend their time, without the stress of relying on inconsistent options.

Whether it’s regular transport to a day program or occasional help getting to the shops, our aim is to remove barriers and promote freedom of movement. For many, this service is a key part of feeling connected and confident in the community.

Access & Maintain Employment

This service helps participants explore, find and keep employment that suits their skills, interests and goals. Support can include preparing resumes, practising for interviews, understanding workplace expectations and navigating supports at work.

These support services are tailored to build confidence and promote long-term success in employment, whether it’s part-time, casual or full-time. We also work closely with employers where needed to ensure the right fit and conditions. For many participants, this service opens the door to financial independence, routine and a greater sense of purpose in everyday life.

Development of Life Skills

Our life skills development support services focus on practical learning in areas like cooking, budgeting, communication and using public transport. These everyday skills are key to increasing independence and building confidence in both familiar and new environments.

Participants are supported at their own pace, with guidance that is encouraging and easy to follow. Learning life skills can help improve problem-solving, build healthy habits and prepare for greater responsibility over time. This support is often a turning point for those who want to take more control of their day-to-day routine with confidence.
